Patient Navigator Program Community of Practice

CLP is a partner with Asian Americans for Community Involvement (AACI) in a federally funded project to launch new Career Advancement Academies (CAA) at three colleges in the Bay Area.  These new CAAs are bridging  students wanting to enter the Health and Human Services workforce to college credit programs focused on the skills and knowledge needed for emerging Patient Navigator jobs.  The Affordable Healthcare Act is redesigning the way healthcare is delivered, and Patient Navigators are a key non-clinical position within medical settings. Three key partners: Cañada College, San Jose City College, and Skyline College have developed curriculum, linked courses, and work based experiences to support students in reaching their goals.  These CAAs are the first step in longer pathways to certificates and degrees which will provide opportunity for students to continue to advance in college and career.
